Please contact your study team. They will be able to:

1. Remind you of your Username.

2. Give you a temporary password, letting you log in again. You will then reset your own password.

Please contact the person who gave you access to the system. This person may be:

1. The Administrator at your clinical site

2. The Administrator at the study's managing organisation

3. Signant Health (use the contact details provided in your contract documentation)
